1. How to affiliate?

Go to https://docs.google.com/forms/d/e/1FAIpQLSchTwFm5-S4u4KqlNwbCnrz1YouBtjLvcg1pDp6QpnmDlhIfw/viewform and fill out the Google Form.

2.  How do you submit your affiliation dues?

Go to https://petaluma.revtrak.net/phs/phs-asb/#/v/phs-asb-club-donation and select HOSA from the pulldown menu. Enter $20.00 as the dollar amount. 

3. Does HOSA provide community service hours? 

Yes, HOSA has a variety of community service opportunities and resources. 

4. Is there graduation recognition? 

HOSA members of multiple years will receive a special sash at graduation. 

5. Do HOSA members go on trips? 

Yes, HOSA goes on multiple trips per year. From state and regional conferences to the Washington Leadership Academy.
